Supported Community Living (SCL)
At REM Iowa, we believe a home is more than a house. It’s where we develop lasting relationships and share our lives with the people we call family.
In our Supported Community Living program, we help individuals with intellectual and developmental disabilities live as independently as possible in the communities of their choice. The adults and children we support live in homes that they secure while our staff provides expert clinical services and opportunities for individual growth during home- and community-based living.
Individualized Service Planning
We take the time to understand each person’s needs and interests. We collaborate with the individuals and their families or guardians, case managers, vocational providers and other members of their support teams to develop individualized plans focused on reaching new milestones. Our Supported Community Living program offers personalized services including:
- 24/7 on-call support
- Medication management
- Community integration
- Management of daily activities
- Structured activities
- Life skills development
- Transportation
We take pride in the quality of our programs. As with all of our services, our Supported Community Living program is supported by a network of quality improvement professionals who ensure the health, safety and happiness of each person we support.
