REM Iowa Recognizes Its Mentors
In celebration of Mentor Appreciation Month, on Thursday, May 14, REM Iowa hosted a Mentor appreciation dinner for Mentors in Eastern Iowa.
Host Home Program
In our Host Home program, the Mentor home provider, the program staff, the individuals’ family and other professionals design and implement an Individualized Service Plan (ISP) to help those we serve acquire skills and access to services while allowing them to participate in community life and activities.
For individuals and families, our Host Home program provides invaluable peace of mind. For the payors supporting them, it represents a reliable, affordable, quality-focused solution that eliminates the need for capital expenses and the worry of ensuring continuity of direct care staff. Services provided in the Host Home program are based on each individual’s unique needs and preferences, but may include:
- 24-hour home-based supervision
- Whole life planning
- Development and monitoring of an Individual Service Plan
- Life skills development/activities of daily living management
- Community integration
We offer the individuals we serve the clinical, medical, behavioral and personal supports they need to be successful. If you’re interested in becoming a Mentor Host Home provider, please visit www.makeadifferenceathome.com, contact us by email, or submit the form below for more information
